Office Policies

We see patients by appointment, 8am to 5pm Monday through Friday. We close daily from 1-2pm for lunch, but feel free to leave a message. We will return your call promptly.

Kindly notify us at least 24 hours in advance to cancel or reschedule an appointment. Missed appointments are subject to an additional charge.

Fees will be discussed during your initial consultation. If you have insurance questions or other financial concerns, please be sure to mention them at that time.

Please notify our receptionist of any change in your circumstances – phone number, address, medications, insurance, etc.
